Waiver of Liability for Participation

As a participant in Jiu Jitsu classes offered by Oceanic Jiujitsu Academy, I acknowledge and fully understand that participating in Jiu Jitsu and related activities involves the potential for risks of serious injury.  I understand these risks may result from the actions, negligence, or failure to act of myself, others, or the condition of the premises or any equipment used. Understanding the risk of harm associated with these activities, I voluntarily choose to participate, and I freely accept and fully assume all such risks.

Assumption of Risk:

I hereby acknowledge that I have been informed of the nature of the classes and that I am aware of the hazards and risks to which I am exposed by choosing to participate in Jiu Jitsu classes. I also acknowledge the possibility of physical injury and voluntarily assume the risk for my participation.

Acknowledgment of Understanding:

I have read this Waiver of Liability and fully understand its terms. I agree to am assuming all responsibilities for my own accord and that Oceanic Jiujitsu Academy hold no responsibilities in the occurrence of an injury.
